How to tell if my Hard Drive is nearing end of life?

I was wondering if my harddrive is failing. These are the reasons why I suspect it so:

1. Windows takes a long time to load. Even after loading, harddrive is really busy for a long time before I can actually use the computer.
2. On average I would say 30 mins for windows to load where hard disk activity drops below 100%
3. Sometimes I get blue screen. I read online that this could indicate hard disk failure. Like this is turning into a once a week occurrence now. This is frequent considering it use to be a rare occurrence

Could you please advise what else I could do to make a case for a new hard drive. Or just a complete factory reset of computer.

I am using Toshiba 500GB Internal Hard Drive (MQ01ABF050). This is a mechanical drive. The drive is 6 years old.
